Digital Permission Slips with Google Forms (2026)

By Jeremy · ·

📘 Part of Google Forms on iPhone & iPad: The Complete Guide

The Caveat First

Digital forms can reduce handling and transcription, but the collection method must fit the activity and the school’s requirements. Review the paper and digital workflow comparison, then have the responsible school staff confirm the required identity, consent and recordkeeping process. Do not assume a typed name or a paper signature is automatically sufficient.

The Form (Ten Minutes, Phone Only)

  1. Student identifier — collect a name or school-approved identifier without displaying the full class roster to every respondent. Reconcile against a private roster.
  2. Class/section — dropdown, if the form serves several.
  3. Guardian full name — short answer, required.
  4. Consent checkbox — restate the specifics: “I give permission for my child to attend the [trip] on [date], traveling by [transport].” Required.
  5. Typed-name acknowledgment — record the person’s stated name and consent. Typing a name is not independent verification that the person is the student’s guardian.
  6. Notes — collect only information needed for the activity. Handle health details through the school’s approved restricted process; do not place them in a general shared roster or notification email.

Choose email collection settings according to the school’s process and provide an accessible alternative. A verified Google email identifies an account, not the person’s authority to consent for a child. Do not equate sign-in with guardian verification.

Distribution and the Deadline

Tracking: the Who’s-Missing List

Reconcile Sheets responses with a private roster, checking duplicates, child identifiers and unresolved consent. Export a separate reviewed file containing only required records, restrict access and follow retention policy. Closing submissions does not lock an archived file or necessarily disable response editing; test those settings separately. See Google Forms for teachers for related workflows.

Our Forms for Google Drive iOS app can help review forms and responses on a phone. Test notification settings and delivery before relying on alerts; receipt of a submission does not verify a guardian or complete the school’s consent checks.

Track returned slips and acknowledge receipt

On iPhone and iPad. Use Forms for Google Drive to review submitted slips on iPhone/iPad. Match the response to the intended activity and keep a separate roster if you need to identify families who have not responded.

With the Workspace add-on. Form Notifications by pipiform can acknowledge a submission to a mapped parent email and notify the organizer. A notification does not authenticate the signer or turn a typed name into a verified signature; choose the collection process required for the activity.

The app and Workspace add-ons are separate pipiform products. Forms for Google Drive is US$59.99/year with a 3-day trial; check the App Store offer for your region. Form Notifications’ limited free mode allows 20 emails/day; paid features and recipient options depend on the plan. See current product pricing before subscribing.

Frequently Asked Questions

Is a Google Forms permission slip legally valid?

Do not assume validity from the form tool or a typed name alone. Ask the school's responsible staff to confirm the process required for this activity, including identity, authority, consent wording and records. Use the approved signature process where required.

What fields does a permission slip form need?

Collect the necessary student identifier, class, stated guardian name, explicit activity-specific consent and date. A typed name records an assertion, not verified identity. Keep the roster private and collect health information only through the school's approved restricted process.

How do parents without the class app or email find the form?

Provide a QR code and readable link through approved school channels, plus a paper or assisted alternative for families who cannot use them. Test access without the organizer's account and avoid exposing student details in public links.

How do I know which students are still missing?

Compare responses with a private roster using approved identifiers. Check duplicates, errors, identity and consent questions before marking a child complete. Contact missing families privately; do not expose the class roster in a public dropdown.

Should each child get their own pre-filled link?

They can reduce typing, but pre-filled values are editable and links can be forwarded. Send them privately, minimize personal data in URLs and reconcile the identifier separately. A pre-filled link does not verify a guardian.

How long should I keep the responses?

Follow the school's retention and access policy. Store a reviewed, dated snapshot with only required information. Stop new submissions and check response-editing permissions separately; closing a form does not make the Sheet or an export immutable.
