Keep the Form Almost Rudely Short
Feedback forms fail by asking too much. The version customers complete at a counter:
- “How was your visit?” — linear scale 1–5. This is your trend metric; never reword it once live.
- “What brought you in?” — multiple choice. Segments the ratings so you know which part of the business each score describes.
- “Anything we should know?” — paragraph, optional. Where the actual insight lives.

Put It Where Customers Already Are
